St. Anthony Regional Mountain Cancer Center is pleased to welcome you as a new patient. New patients should be prepared with the following information: Please be aware that visitors under 18 are not allowed into the hospital due to the H1N1 precautions. This policy also applies to the cancer center and you will not be able to bring children with you to appointments. Thank you for understanding.
- Completed Patient Information Packet - For your convenience and for a speedier check-in, we ask that you download and complete the patient information packet and bring it with you to your first visit. (Please note the patient medical history portion is seven pages.)
- Necessary Referral - Many insurance companies and HMOs require their insured customers have a referral from their primary care physician (PCP) before they can be seen by a specialist, and those companies will not allow us to see you for your first visit without a referral. It is your responsibility to check with your insurance company to determine if a referral is necessary. If you need assistance with the referral, please contact us and we can contact your PCP to arrange the referral.
- Advance Directives - As part of our compliance with the Joint Commission on Hospital Accreditation, we ask that patients who possess an advance directive (such as a Living Will, Colorado CPR Directive, Medical Durable Power of Attorney, etc.) bring them with you on your first visit so we can make a copy to include as part of your medical record.
